I'm not sure about medical revolution, but I know it has something to do with advancments in medicines, which is why the death rate drops in stage two of demographic transition.
CBR is crude birth rate which is the number of babies born each yearfor every 1,000 people and CDR is the crude death rate which is the number of deaths each year for every 1,000 people.

TFR:The average number of children that would be born alive to a woman (or group of women) during her lifetime if she were to pass through her childbearing years conforming to the age-specific fertility rates of a given year. This rate is sometimes stated as the number of children women are having today.
